3. Once you click download, a save as window will appear. Save this file to your computer
4. Open the file from your computer where you saved it, type information on to this form
5. Once finished, save completed form to your computer
Nominations must be submitted by March 7, 2025 for consideration to honor at this year's event (August 23, 2025). Nominations will remain in the selection pool for three years.

The George B. Carlisle Distinguished Service Award is the highest award the foundation gives to a community member. The nominee must have made exceptional contributions that have benefited the Clear Creek Independent School District; is widely known as a community leader dedicated to excellence in education; exemplifies public service, and has served the district, the students, and patrons over an extended period of time.

The Distinguished Alumni Award is presented to a Clear Creek Independent School District graduate who has excelled in his or her chosen profession and has made a significant contribution to his/her community.

The Dennis Johnson Memorial Small Business Award created in 2013, is the award the Clear Creek Education Foundation awards to a small business or franchise in recognition of its owner's exceptional contributions that have benefited the Clear Creek Independent School District. Nominees should be widely known as a community leader dedicated to excellence in education and should have served the district, the students, and families for a lengthy amount of time.

Valor implies honor, dignity, bravery, morality, and heroism. It can also suggest strength of mind or spirit that enables a person or persons to encounter difficult situations with persistence, determination, and personal courage, often in the face of great risks. A nominee for the award must be an individual or group that has demonstrated one or more acts of valor that have benefitted Clear Creek Independent School District (CCISD), its staff or students. The nominee may also be a CCISD graduate or employee whose act(s) of valor reflect positively on the district and have benefited others.

The Clear Creek ISD Board of Trustees is now seeking nominations for the 2025 Citizen of the Year. The award is given each year to honor an outstanding community member for their support of CCISD programs and activities.
Recipients eligible for this award must meet the following requirements:
- Demonstrated a commitment to Clear Creek ISD through volunteer efforts.
- Contributed significantly in ways that have benefited the entirety of Clear Creek ISD.
- Maintained a reputation across the Clear Creek ISD community as an individual with high standards and dedication to excellence in public education.
- Voluntarily served the district and its students and patrons over an extended period of time.
